I finally sorted this ioctl thing out.

It was ALSA.

I won't bore you with all the details, but it was some kind of arcane cruft 
problem.  Probably something with the switch from 0.9.0rc6 to 0.9.3a.

I sorted all that shit out by compiling a new, virgin kernel with a new set of 
modules, and everything seems to be behaving.

I hated to leave my trusty old kernel behind though.  I ran that thing for 
ages, and it never stumbled.
