No, never. But I was reflecting yesterday on the number of times our code looks up a tool by name and then blithely follows the returned pointer without ever checking it's not NULL. Hence the crashes. We've had many similar types of crash in the past from looking up the instrument for a particular track and then using it without checking it actually exists, too. There's obviously a class of error there that we could do with a good general way of avoiding -- but offhand I can't think of one, except due care and attention of course. No idea why it would lose the tool in the first place though.
