OK, I've edited the hardcoded values (for new devices) so as to hide 
them from the instrument parameter box, and to change the default 
expression value to 100.

Now I'm about to run a script to edit all the .rgd device files in the 
library and make the following changes to each:

 -- If the Sustain and Expression controllers are still in the default 
instrument parameter box positions and still have the MIDI controller 
number 64 and 11 as in GM, mark them as hidden from the instrument 
parameter box.

 -- If the Expression controller still has default 0 and MIDI controller 
number 11, change its default to 100.

Please, if you object to this -- either because you think it's generally 
wrong, or because you contributed or use a particular .rgd file for 
which you think it shouldn't apply -- speak up!
