In my experience, requiring users to use their real names and requiring
registration where you send them their password by snail mail greatly
cuts down on the spam.  You still allow anyone to register, but they
must provide a mailing address and possibly a phone number.  That is not
a problem for a legitimate user (and might actually help build a sense
of community by initiating some more personal contact between people)
but will generally filter out the spammers.
