I have converted DOS real time programs to RT-Linux and found it to be a fairly easy 
task.  I did not have to change any of the computer hardware to make it work.  
However, I did have to deal with the permissions to get direct access to  the I/O 
ports.  There are two ways to do this.  One way is to run as root and the other is to 
set the uid.  There has been some discussion of this before and it should be in the 
archives.  I would point out that if you have PCI cards there is considerable more 
interface software involved than with ISA cards.  This has been discussed here as well.
