True, I have not. I've been lucky I guess.

So if SQLite was a mistake, then tell me, "What database should Rails 
use by default?"

A: SQLite
B: MySQL
C: PostgreSQL
D: None of the above

"Who's opinion should count?" Maybe having no database as the "default" 
would be the right choice. In that case we have the next best thing: 
SQLite. ;-)
