Yes, "Add row" appends a row. If you want to insert a row elsewhere use the green "+" icons beside each row.
Whereas XLS Decision Tables are split into two sub-tables the web-guided one does not need the extra information. "RuleTable Eligibility Rules" should simply be the name of the asset. Functions, Declared types, etc defined in an XLS Decision Table are simply extra assets in Guvnor.
