I received the same PBL newsletter/catalog and read of the problems with some 
interest.  Seems to me that most all of the problems stem from one fundamental 
cause -- the lack of receiving inspection.  Parts were accepted from one vendor 
and passed along to another supplier in the manufacturing process without a 
thorough verification for correctness and accuracy.  The supplier used the 
parts as they came and passed the end result along to the next step in the 
manufacturing system.  By the time the problems are noticed, a long time has 
gone by and many steps in the process have to be repeated at a considerable 
loss of time.  The moral of the story, in my view, is to closely 
inspect/measure ALL the parts before accepting them and using them in the next 
step.  Yep, it takes time, but it saves time in the long run.  Just some 
thoughts for any budding manufacturers out there.  Cheers...Ed L.
