I can't say anything about the inserts as I haven't run accross any yet. If you have an issue in mind I can look. As far as copying goes, my approach would be to use a program like Printkey to capture just what you want of the screen, then save it to file as a gif or jpeg. After doing all the pages of an issue, they could be easily converted to PDFs and combined into one file all at one time by Adobe. It wouldn't take too long to do one issue, but I have better things to do than to invest the time it would take to do 100,000 pages. This would be legal as long as it was only for your personal use and not sold or given away to a third party, which is why Kalmbach made it difficult to do.
