Don and all, This problem does, indeed exist in HO... expecially on the Athearn Genesis F-3 models... but the # 233 gear box that comes with Kadee's 30-series couplers does not solve the problem. It also interferes with the rear trucks on these models (and possibly the F-unit models from other HO manufacturers as well, but I can't say from experience). Kadee makes another gear box, the # 252, that is very similar to the # 233, but it also does not solve the problem (at least not on the Genesis models).
However, I have noted that the SHS gear box is almost identical in size to Kadee's # 233 and # 252, so it's easy to see why it solves the problem in S scale... it is basically an HO-sized gear box solving an S scale problem.
