So I think this patch is ready for review! Apply trac5249.patch.
Test coverage is up to 100%, and I renamed the "hole" parameter to
"region". This implementation uses only the IndexFaceSet based backend,
but it is fully functional. I would like to get this into Sage, and then
we can work on integrating MarchingCubesJVXL again. Robert Bradshaw had a
good suggestion, which was that we should make ImplicitSurface more
generic so that it can accept a user-specified array of voxels and
triangulate that. I can open a ticket to address these potential features.
