 Would it be acceptable that `biseq_setitem(S,index,item)` assumes that
 `S[item]` is zero before the assignment? This condition holds in the two
 applications I can think of, and deleting a previously existing item would
 be a waste of time.

 Of course, there could also be a function that deletes an item.
