I fixed (4) in commit

(1) This will be fixed in the next version of the spkg.
(2)-(3): I couldn't replicate this. Can you give a precise sequence of
steps to replicate this? Here's what I did to test it:
1. Log in as admin
1. Create a new user under the Settings | Manage Users
1. Log out and then log in as the new user
1. Change the user's password
1. Log in again with the new password
Everything seemed to work fine.
