With v4 the send options will change and probably you'll be able to solve
the problem differently at that time.
You will be able to say
SEND PAGER when host is 3 times down and every 25 times
on the 3rd down cycle a page will be send
and on the 28th one too
and on the 53rd one too
...
(this is just a sample, the 3 and 25 are ofcourse not hardcoded but
parameters you'll be able to set per entry)
that way the current active person will be get a reminder, but if in the
mean time due to the schedule somebody else is (also) active he/she will
also get it.

maybe it's possible to solve the problem at this way:
The person's scheduler is splitted to full hours for smtp and pager
notification.
Every full hour a routine can query the status of the hosts and 'simulates'
SA a status change when the status is down.
