I'm not sure if this helps your case but I have a large number of alerts
that when they alert they will every other check schedule unless they are
fixed or put into maintenance, if an engineer receives more than 2 he will
respond, what happens if you set yours for this and the one engineer puts
the down into maintenance then the alert will stop, if the other engineers
don't receive an alert after say 30 mins after the initial 2 downs then
either it's been fixed or put in maintenance, I presume you have a log when
an engineer does some work so all the other engineers can look to that to
see what happened when they come on-shift etc? I know this isn't ideal but
may help

What is manualy?
You can put an entry into maintenance via the interface/telnet/ssh (in v5
via an alert too). And if you would use saWeb then the telnet option become
very common, since saWeb uses the telnet interface...

That would mean a huge change in the way the alerts are
checked/generated...don't think this is something that will come any soon
into the product.

What we are however thinking about is some kind of generic alerts were you
can generate an alert when a host is more then .... (minutes/hours) in a
down/maintenance state.  This alert would be for all entries (or maybe per
group).

Well, after a check is reported as down, and the alerts go out, I'd like to
be able to set an additional alert to go out if someone comes in and places
the check in maintenance at the GUI front end (as opposed to if it gets
placed in maintenance by a schedule).

We had a case this weekend where the primary tech put a downed server into
maintenance, but didn't notify anyone, so the 2 other techs on the call list
showed up at the office to check the issue.
