You probably know this, but you can reach the 2003 "machine" session via
RDP as well, with the latest TS client. Use the syntax;

   <computername> /console

in the server to connect to dialog. You need the version of MSTSC.EXE
(and mstscax.dll) that come with 2003, maybe the XP one works, I'm not
sure. Then I guess you could disconnect kvm for a little more physical
security?

I recently corrected a similar issue on two Win2003 servers.  The fix
was to set the SA service to run as system* with desktop interaction.
All of this has to be done on the machine, not a terminal services
session, and then the service needs to be started while sitting at the
machine.  Then I lock the server rather than log out.
