I am not sure why you replaced your GPL version 2 with the GPL version 3 in
the new tarball, when the GPL version 2 was fine.
Now you have GPL version 2 or later license blocks with the GPL version 3
license. I don't think that will work.
Which license do you wish to have?

I guess because it is being packaged up into a deb package and you placed the
copyright and license notice in a file called copyright.
What about the CHANGELOG in the top level directory? It is usually missed to
place a copyright notice in it, but it is recommended by GNU.
What about the *.form files do they not need a license notice? Are they
trivial or derived (generated) from another program?
