The Shoutcast DNAS server is available free of charge for most
platforms from http://www.shoutcast.com/broadcast-tools

If you need a server up and running somewhere I will be happy to host
one for you. (linux)

Unfortunately it's closed source...

As for 2.x clients the only one I'm aware of today is Winamp, but
there seems to be a lot of interest on the winamp/shoutcast forums,
and Shoutcast remains one of the largest stream directories on the
internet yet they only accept listings from real Shoutcast servers.
(even though Icecast is shoutcast 1.0 compatible, the SC directory
will reject icecast servers). The fact that their stream directory is
tightly integrated into WinAmp means that getting listed in their
directory is very valuable to streaming stations.

I would guess that Shoutcast is easily the most popular stream hosting
software, based on the vast number of budget shoutcast stream hosting
companies out there.

The Shoutcast DNAS 2.0 server was initially released in beta in early
2010 so it's been around a while now.

Shoutcast has always been a bit of a pain for me as a stream host
provider as it required a separate process per stream instance (and
was closed-source), but 2.0 has added the concept of icecast-like
mountpoints which I would really like to utilise.
