I am a little confused with your confusion :) but will try to help.... Basically you can start simple - just set up your roles first. Create a farm (not app!) and setup all your roles based on the initial roles that are available from scalr.
After setting up the roles you need to synchronized to all in order to save your changes. After this point you have images that reflect your requirements but they cannot communicate since no names are assigned (you can use IP addresses to test communication if you really need to). Now you can set your application on top of the farm. Create a new application and it will ask you if you want a totally new or from an existing farm. Use the farm described above. The application has one name, which is the domain name - for example ' mydomain.com'. You need to only set it up once in the application name. It will also ask you which roles will create A records in the DNS. You can add later more records in the DNS if you need to. This is all you have to do! Names are now generated automatically for you in the DNS. Chec the /etc/aws/hosts directory on any of your live instances - it has directory for every relevant name and in that directory you can see the IP addresses assigned to that name.
