I have implemented the feature much as first described but with it activating whenever the file has no known lexer, not just for files without extensions. Files that start with "<?xml" are treated as xml. This doesn't meet everyones needs but it does work automatically for a large set of files in a way that I don't expect to choose the wrong language.
