The short answer is yes, you would have to create a whole new gear box. There's always a virtual cursor on a braille display. But there is only a cursor in edit controls on the screen. There is no cursor in menus, buttons, check boxes, etc. For example, when you activate a menu bar and are on the File pull down item, there is no cursor and no single character which has focus. The word File has focus. When you're on an OK button no character has focus and again there is no system cursor. That control has focus.

You might want to take a look at the Text object for a start.
