Yes, it is reproducible, but I don't know exactly what circumstances trigger it I'm afraid. Today I managed to reproduce it immediately after Empathy started. A video demonstrating the problem is attached. I'm not sure how useful it is. Basically I just double click on one of my contacts and nothing happens. This issue seems to be resolved for me, I haven't seen it in a while whereas it was previously very intrusive. I'm running Unity 2D and previously experienced it under that environment. I haven't changed anything specific to this issue besides accepting regularly scheduled updates. I had continued to experience this in December. It can't be consistently and reliably triggered. There are times when I don't see this bug show up at all. I've never seen it show up with a short PDF, however by keeping evince open for extended periods (sometimes days) while viewing a long document and switching between windows, the bug tends to appear at the point where I either ALT+TAB to the evince window, or navigating the document with the mouse wheel, arrow keys, or j, andr k keys. Read a long document, say a book, while making notes in another window and you'll eventually see the error. I strongly suspect this is a unity bug and not an empathy one, I've had the same thing, and then the window has appeared after I for example switch workspace. I have found a workaround: for some reason, the password dialog wants to only show up on one of the monitors, but only the one on which the mouse is when showing the password dialog is displaying stuff (stuff being anything else besides the top bar). So what I do when I don't see the password dialog is to press escape once, then move the mouse to the other monitor, and press escape again. This may have something to do with the fact that I'm using nVidia drivers (there are always some annoyances with it and two-monitor setup). However, I think most users won't look at it as sensitivity, but how long it takes to reveal the launcher. But if I were surprised – I actually thought it didn't work at first – then I think others might too. At the very least, this should be tested on innocent users.
