Just add the bibliography to the end of your "general lyx file" by doing
Insert >> List/TOC >> BibTeX Bibliography. You then get the option to choose
the references database etc.

If you google for "lyx thesis templates" you will find a few ready made
options. One of them might be of use, and probably simpler. I'm using the
"uofsthesis" template for my thesis. These templates tend not to be exactly
what you require, but changes in the layout are easy to make in lyx.
