The writing is very neat.  The document is called "Per-filha-çao" the act
of assuming his/her own child in eng is AFFILIATION which is rooted in the
latin.

Antonio Baptista is the Father and is providing the documentation to the
priest to assume responsibility of his daughter Ana.  The subsequent pages
states that he wishes the child to partake of his inheritance etc.  talks
about him being single and the child having been recorded on the baptismal
records; born of father "oculto" hidden/unknown to the widow of Francisco
Bernardo.

It's not an adoption. The father is steping in with a petition assuming
that he is Anna's "natural" father. I suppose at the time of the baptism
she was "natural" daughter of Maria Luiza, and "filha de pai incognito".
