In setting this up - should all the IP's be the same?

You are using 127.0.0.2

In the site: http://www.cluecentral.net/rbl/showcountries.php

He suggests different IP's for different countries.. Or am confused
about what the IP does or should be?

The instructions aren't very clear.  Although it looks like different IPs 
are returned for the different countries, the zone files show them 
returning 127.0.0.2 in all cases.  My guess is that the country zones 
(ca.rbl.cluecentral.net, for example) all return 127.0.0.2, but there is a 
master list (perhaps just rbl.cluecentral.net) that returns a different 
code for each country.  But I could be wrong.

My advice would be to use *, like this, so that any result will cause the 
test to fail:

 CHINA  ip4r  cn.rbl.cluecentral.net  *  5 0

The way that I've done it is as such:

CHINAip4rcn.rbl.cluecentral.net   127.0.0.2   5  0

in your global.cfg.  Then, use the CHINA test as you would any other test.

Do this for each country you wish to block.

 that have 1) Huge numbers of open relays, 2) Spam companies that send

I hate to block entire countries from sending Spam, but there are a few 
that have 1) Huge numbers of open relays, 2) Spam companies that send
