This is a known issue on Nvidia driver for a long time.
Originally it only happens on Unity Launcher, now it also affects desktop

To solve/workaround this, the background needs to be redrawn upon

Thank you. I fixed the broken symlinks.

(By the way, there are other broken symlinks for other apps I didn't
create.)

I couldn't find anything that used the system-file-manager-panel icons.
I wish we had something like https://codesearch.debian.net/ to make this
easy to check.

I did find LP: #553256. It may be obsolete because Nautilus doesn't use
a separate dialog window for file operation status (like copy, move,
delete) any more; it's included in an integrated popup window on Ubuntu
16.10+.

Had this issue since a recent update. I'm using a 16.04. All worked
fine, and after an update the /run/user//gvfs stayed empty. I added
my user to fuse group, killed gvfs and restarted nautilus, and it
worked.

The apps/ ones are broken symlinks. I didn't actually check this but I
should think that's a problem and they should point to system-file-
manager.svg instead.

Are the status/ symlinks necessary? I thought that these were used for
the panel - who requests those icons and how do they do it?

You appear to have packages installed from xenial-proposed and xenial-updates.
This would indicate that -proposed was enabled at some point in time in the 
past on this machine.
You need to downgrade packages.

What does downgrade result in?

E.g.: $ sudo apt install libsystemd-dev=229-4ubuntu16
libsystemd0=229-4ubuntu16

And similarly declare any other packages for downgrade, if above still
complaints about systemd related packages.

What I think is happening in our case:
Since no ExecStop= was specified, systemd will send SIGTERM [...]
Details: https://www.freedesktop.org/software/systemd/man/systemd.kill.html#

KillMode is "process" in the service file.
That means "If set to process, only the main process itself is killed."

So in this case it relies on that being forwarded to the child processes.
That takes time.
If not waiting for it to be "complete" the following restart will send the next 
SIGTERM and this eliminates the (already in cleanup) main proccess before it 
can distribute the TERM to its childs/siblings. This is our error state.

In this broken state
 Main PID: 10600 (code=exited, status=0/SUCCESS)
Our mode of KillMode=process might have special handling and kill all of them 
(since there is no main to kill). That is the cleanup, which gets it back to 
work again.

Since the service files in both (X/Z) cases are the same I wonder if
there is a systemd change which fixes this by some sort of waiting for
the signal to be handled (e.g. waiting for the MainPid to go away on its
own).
