Yes, gnome-shell should still be smart enough to put the right and down arrows in alignment. I have hibernate-status-button and suspend-button and that's probably user menu is extended since the buttons don't fit. Should I still file an upstream bug? It looks like this is a gnome-shell bug revealed by one of the many extensions you have installed. But I don't think it's a bug in any extension. It should be fixable in gnome-shell.
