I have not, and I bet the other 99.9% of our user base did not too. 
Looking at the slippy map screenshot, it is not even what I mean ;)

Just a basic 'linestyle' outline will give the user a direct idea what 
is present (the world) in front of him and how it is presented (white 
lines, black colour) to him.

In Merkaartor, you just have to activate one of the image background
providers, e.g. Mapnik, to have a full map in the background...
You can then move freely to wherever you want, CTRL+D Enter and you have
the OSM data of the current view...

Enabled by default? It's a matter of choice. I'd rather not have the user
use bandwidth without him wanting to.
Show what we have? There is a Zoom all function which adapt the view to
show all downloaded data.


 But in essence I was talking about a vector representation, not bitmap
 perse (if bandwidth is any limitation).

 I'm afraid there is little chance to see that in Merkaartor (not worth the
hassle vs. what we already have, IMHO). Maybe JOSM...

But isn't is much more easy? Something like downloading a static OSM 
file with administrative areas from a static webserver (one time) and 
present that in the main window. Just as indication for the user what a 
map looks like in the specific application.

If I look at the screenshot of the josm slippy_map_chooser plugin, I
can see the boundaries of countries.

Please be sure to do it in such a way that newbies to OSM/Merkaartor
don't get easily confused when they see admin data pre-loaded.

I would stick with the slippy map as implemented in Merkaartor. And
maybe, (really maybe) enhance the highest level map with country names.
(can be special nodes, easy to maintain, easy to render and easy to get
from the OSM database)

In my opinion, a person that is not able to find his own country on a
smoothly moving worldmap should not be mapping at streetlevel.
