I am seeing usb supplies with 5v and 9v output.  For example, the usb
supply that came with my Samsung galaxy or Arlo camera.  The current
rating might  be insufficient.

same for me with speakers. Hadn't thought about my TV.  It's the one of
the last high quality Plasma TVs made (65 inch panasonic with 3D) and
probably 9 years old.  Fingers crossed have had no issues.  Then again,
the power supply is built in rather than a power brick.  Speaking of
'boomers' I have a 1957 Hallicrafters Short Wave Radio that still works.

I've had two Duet receiver power supplies fail over the last 12 years.
The last time I had a failure I bought one of these 'cords'

and then I can just pick up another USB wall charger. I did have a wifi
card in a receiver fail and that was also replaceable. Any time I need a
player now days I just build up another PicorePlayer. These PCP's will
keep the LMS ecosystem alive for years to come!

My TV is also over 10 years old - it was a top Samsung 52inch LED model
at the time and cost me dearly, so no need to upgrade it yet... but
it'll probably blow out soon.

Then again I have speakers that I have owned for nearly 30 years... and
they still sound super sweet when I go through occasional rotations.

..at least for me it is the dang Logitech power supplies every friggin'
time. So don't assume the box broke. 

I am lucky enough to have several of them around so can easily test with
other power supplies. And... voila!... it's fixed it every time. Never
had another issue with the stable of Squeezeboxen I have... about 5 S3s,
2 Touches, 4 Booms, 1 Radio, 1 Duet. Always the power supply.

I can't recall worse power supply quality in any other product, but in
all fairness I am not sure I have any other piece of electronics in the
house that I still use every day after 15 years or so...
