Can't you just use Acrobat Professional 8? It translates the PDF to Word as
text insets, which you then change to pure text. It works for me.

Faults
Internal xrefs and hypertext links usually get broken. 
Sometimes, some graphics get dropped as well.

Exactly what I thought, Caroline.  Doug, can you select the body text in 
Acrobat using the Select Text tool?  If you switch to the Select Image 
tool and click on the body text, does the whole block between headings 
get selected?
