Materia Magica actually has a custom addon pack that we have bundled with MUSHclient, so that the setup is quite smooth. We have sounds built into this, but they tend to be more informational and environmental cues rather than atmospheric sounds or background music. You might try Materia Magica. It's not directly based on ADD, because like Dennis said, those rules don't make for a great computer game. But we have the familiar fantasy settings of dragons and magic, warriors and quests. I'll don't know if Materia Magica will be sufficiently different from Alter Aeon to suit your mood, but it might be worth a try.
