The hoop (no idea what it's official name is) has a horseshoe-like
shape. You open it and place the fabric so that when you close the
hoop the fabric edge is across the opening between the tips. Friction
holds it closed.

working on handkerchief edges. I wasn't convinced that it would hold
taut enough, but after testing it does appear to work just fine.

It sounds like something that could be used for buttoning.

As for the original mystery item, it looks too small and too smooth to 
be used in hair. It would need teeth of some sort to hold a roll of 
hair.  I thought, from its size it might be used to hold open a sleeve, 
pant leg, sock or lady's boot, perhaps for repair work or to let it dry. 
But again, it is very smooth and I doubt it would stay in place. And 
shoe shapers aren't made like that.

Yes the sleeves are extra long and the tight wrist keep them from falling over 
the hand. I'll bet they are 2 pieces with a closure on the outseam that closes 
with something like hooks and eyes. The upper parts are indeed falling 
concentrating the ruching at the narrow bottom. If the two piece sleeve is also 
curved like a narrow sleeve would be cut, the bottom of the sleeve will be 
slightly on the bias. This will help with the rushing as well as the tightness. 
(also, it's not a very good painting, His right hand with the pistol is smaller 
than the left and I think the foreshortening of that right arm is not quite 
right and looks too narrow. Not so his left.)
