Hi Susan,

The prayer is said on Yom Kippur.  I have enclosed the translation from the 
Artscroll Machzor (I  don't have access to a Conservative one).

Mankind, Mankind and I remember the ending - yesofo l'afar but I can't 
remember the coming from dust part ([something] m'afar that comes before the 
ending).

The section your patron is asking about is actually part of the piyyut
Un'taneh tokef, which is a silluk, leading into the Kedushah  (Kadosh,
kadosh, kadosh..). In Mahzor Hadash it's on p. 284-5. In the Goldschmidt
Rosh HaShanah volume it's page. 171. It begins Adam y'sodo me'afar/v'sofo
le'afar - Humanity - it comes from dust/and its end is dust. (Your patron's
memory conflates the 2 half-lines).  The end of humans, our finite life, is
contrasted with the eternal existence of G-d.
