After the initial compilation attempt, you can open the generated .tex 
file (lurking in the buffer directory) in a text editor and search for 
any mention of verbatim. It could be that some module or funky document 
element is using it. If there's more than one occurrence, the location 
of the error in the document log window might help narrow down which one 
triggered the error.

wonder why it suddenly decided to show up each time I use the chord, C-x h.

Regardless, when I close the error window and repeat C-x h pdflatex happily
compiles the file.

Has anyone a thought about why the error would appear if the environment

Use custom and fill the box with the path to your favourite pdf
viewer.  You may not need the full path, if your pdf-viewer is
directly accessible.

    Select there the relevant PDF output, e.g. 'PDF (pdflatex)'
    and select the Viewer to what you desire.

Not applicable. But, about 15 years ago I used PSTricks to create a
scannable form. Took time but worked.

These 'forms' are not like a voting ballot. One is a checklist of compliance
with the state's environmental policy act, another describes the project's
compliance with the local critical areas protection needs. And so on.
Question and answer type forms. Then there's the biological
evaluation/assessment which requires addressing specific issues.

Because I no longer have a typewriter, use windoze, or want to write
manually I recreate the 'forms' in LyX.

Sometimes the specific appearance matters, most of the time it's the content
that matters. As long as all information is provided in the same sequence
the agency would accept it.

that is the problem: You have TeXLive installed in /usr/local/texlive/2022
and also the Debian specifc one in /usr/share/



but I had removed all the tex related package from /usr/share so that it 
it in /usr/local only.


Strange.

Perhaps I should work as a different user on my notebook and start anew 
with lyx. I am also inclined not too use the Lyx2.4dev any more. It took 
me so much time and frustration. However, before doing it, I should know 
how to get the lyx2lyx conversion in order to downgrade my 2.4 lyx files 
to the 2.3x

First, if the forms will be OCR'ed, you'd better have the right font
and spacing. Otherwise, it's pretty easy...

Your layout file defines environments and commands. Commands are the
basis of character styles, and can be used for other things too.

So invent a stylename (environment or command) for each appearance you
see on whatever form you're imitating, and then do a little trial and
error with the styles until yours looks exactly like the sample.

I haven't discussed the nuts and bolts of custom layout grammar. If you
need that, give me back a shout.

I have recreated a number of forms required by the local Medical
Industrial Complex :-)-O using tables, judicious spacing and other
trickery to achieve reasonable similarity which has been accepted in
each case.

I usually use Komascript but have a few legacy forms that I created in
Memoir and they are not broken.

It's a lot of manual labor, even only once:-)-O and you can template
(variables like $$SURNAME$$) with Perl (or for the younglings Python
:-)-O) if you want.

The really complicated and colorful forms I do in LaTeX with the PDF as
the background and then use something like \usepackage{lpic} so I can
place the variables right where I need them, usually incorporating
them into Perl scripts.

Using Pashua for simple forms and Platypus to create (simple) Apps on
the Mac makes this quite powerful.

Usually these forms change over time but in particular the lab I use has
overlooked the outdatedness of my forms (probably because they are happy
not to have to try to read my handwriting :-)-O)

Thinking I'm on the right track, I'm copying text from the PDF form
with questions emphasized (to visually separate them from the filled
in answers).  I'm using sections* and subsctions* to follow the
agency's formatting.

Will save in ~/.lyx/templates when finished.

Recommendations still welcome!
