As I remember, there's an option that you need to change in order for function
keys pressed without the FN key to be recognized as funciton keys, if you're
using a laptop keyboard.

Hello Paul,

As far as I know, the only way to launch VoiceOver is with Cmd-F5. However, 
sometimes VoiceOver gets muted so it’s a good idea to create a shortcut in one 
of the commanders for the commands “Mute Speech Toggle” and “Mute VoiceOver 
Toggle”. These commands are in the Audio menu of the Commands Help menu.

yes you can automatically start voice over. go to your system preferences then 
users then log in, then check voice over on start up.

Could it be that your function keys are set to hardware instead of software? 
Try hitting fn+Command+F5 and see if that works.
