Yes, I have noticed that. If you are wanting to demo something, you are better 
off using a. Patch cord anyway for a number of reasons.

I was able to do this using my Bose speaker, but while VoiceOver speech was 
routed through the Bose speaker, VoiceOver sound effects continued coming 
through the iPhone’s speakers, and VoiceOver’s responsiveness was extremely 
diminished, and the sound effects volume was abnormally amplified.
To me, this was not an acceptable solution. .

In VoiceOver Utility -> Sound, you can choose the audio output device to 
be used by VoiceOver. This is independent of the default audio device 
specified in System Preferences.

I do remember encountering this problem some while back. If I recall, the 
problem has to do with the Bluetooth protocol being employed by the device, and 
there’s nothing that can be done about it from the accessibility standpoint.

I believe this is a function of the firmware in a particular model of Bluetooth 
speaker.
Beginning with iOS 13, certain brands of Bluetooth speakers began to not play 
VoiceOver through the speaker while playing music or other content.
For example, I have a Bose Bluetooth speaker, that when playing music on this 
speaker, VoiceOver speech continues coming from my iPhone, while on my Jawbone 
Big Jambox speaker, both music and VoiceOver speech come through the speaker.
I am not aware of a way to change this behavior on either device.
