Hi Debra,

I'm going to try to help you with Dragon, but I need some information you
may have provided previously, so forgive me if this is redundant.
Specifically, what microphone are you using (make and model) and is it
plugged into a USB port or the analog plug(s) for your soundcard
(microphone and possibly speaker/headphone jack)? Having a decent quality
microphone is a must, and depending upon what it's plugged into,
adjustments may be possible to improve performance. If you can provide this
information we can go from there.

Regarding your nursing home situation and inability to summons assistance
via a call light/bell, many hospitals and I would hope nursing homes have
sip and puff switches that can be swapped with the call light/bell. Has
this been explored as a possibility? This website has some options:

I'm very sorry for your circumstances with Dragon and the nursing home. I
know I get frustrated with Dragon and I have regular assistance nearby the
majority of the time.

Sometimes when I seem to be having difficulties with recognition of my speech a 
little bit, this happens:
