I have a set of those Simplex shifters, and will gladly sell them along to 
anyone who might be interested (along with the Silver stem mount where they 
currently reside, if you'd like). They're the most beautiful downtubers 
ever, but I think Silvers shift better.

 Quality NOS sets are harder to find these days, but if you can find a set, 
 I prefer Simplex Retrofriction to Silvers.

I prefer friction. Maybe because it's what I cut my teeth on but to me it's 
simple, feels good and the end results are the same. Joes spot on on this, 
the Riv shifters are smoth. Ratchet or not, I'll take friction. But 
that's just me.

 I would shift a notch but still have to try and trim a half notch or so to 
 get it right. So finally I stopped and took the shifter out of index mode 
 and into friction mode. Viola! It shifted like butter! What a wonderful 
 relief. I had tried index mode once before but only briefly. This time it 
 is great. I'm not sure if the indexing is wearing out on the shifter or 
 what. I would tend to suspect my not so great mechanic ability, but the 
 great thing is, it doesn't matter. I loved the friction shifting. I think I 
 may never go back. This is one more thing that Grant is right about. I

FWIW, after using Simplex Retrofrictions on the new 9 speed Dura Ace 74XX
drivetrain on the Ram for a couple of months, and after having used Silvers
(bar end) for several years on the Fargo, both 7 and, for 18 months or so,
9, I have to agree that I find the Silvers more precise and easier to use
with 9 speeds.

That may explain our different experiences.  The largest rear cluster I've 
ever had is 7.  Have never had a triple crank.

Have you checked rear derailleur hanger alignment? This might be the 
problem with indexing. 

Otherwise friction isn't that bad.

If you like Shimano index, you'll *love* Silver Power Ratchet. That 
sorta-clicky thing it does when you pull back/up on the shifter somehow 
magically gets you to the next cog with almost no trimming. Ride that for a 
while then hop on an index bike, and you'll feel like you're shifting by 
hitting the derailer with a hammer. I still like and use both, but Silvers 
are swell.

I had pretty much the same experience with the tandem.  shimano BEs  After 
replacing the rear derailler and chain it would hang up in the middle gears 
and then jump two cogs.  After a few rides and much tinkering I switched to 
friction and rarely miss a gear.  Much faster now too.
