We reflex if LA >2 also. Usually 3 hours from initial draw regardless what 
result is.  This helps to not miss in repeats  most of the time.

We do not wait for a specific time.
The nurse calls the respiratory therapist as soon as the fluid bolus is 
infused. RT uses the ePOC to do the point of care initial lactate, hangs a 
laminated sign reminding the nurse to call when the fluid bolus is complete for 
any lactate >2, and then returns to do the repeat lactate when called.

The repeat lactate lab order is timed for three hours which means the actual 
collect time is somewhere between 3 and 4 hours.

We use 4 hours. Our intensivists think that any time earlier (i.e. 3 hours) 
isn't enough time to see clinical significance.

Our Lactate  order is Stat and then q 3 hours times 2 no matter what the 
initial lactate level is. We have found (with the help of Wolters Kluwer Health 
and the Point of Care Advisor) that about 5% (4.7%) of our patients with an 
initial lactate less than 2 go on to develop septic SHOCK.
