If it does not happen in a new profile then depending on how old the profile is you might have some junk in the permissions database. The mozilla migration code did not do a stellar job in 2.39. It just copied everything it found in the history and made some guesses. Check with the Data Manager or just start with a new permissions.sqlite in the profile. Select Load all images. That's the one, thanks, Ed and it seems it has fixed Larry's problem, but not Cecil's!
