This 
may be a bug in M-Tx, and I can't help with that. But when I process the 
following file in PMX 2.40 I get exactly what I expect:

I made two alterations to the code:
1) I exchanged Piano by another name, not predefined and added
 Systems: 1 Pages: 1
2) rb4d was changed to the normal order r4db.
Then M-Tx (I use the option to suppress warnings) produced a normal
pmx and eventually a dvi-file which shows the expected typeset.
Therefore, there is no bug in M-Tx.
I did not experiment which unusual coding produced the error, however,
I justed changed to my customary code. The [ ] are unnecessary.
