Bug description:
  I upgraded to Ubuntu 16.04 yesterday, I did a clean install while
  preserving the home directory which is mounted in a separate
  partition. Even during the installation I had constant volume
  notification bubble appearing on the top-right corner of my screen, it
  would come and go about twice every minute. I just thought it was due
  to installing hardware drivers or something but the problem persisted
  even when I booted into the freshly installed system. I did some
  searching and two users reported the exact problem. And one more
  suggested this happens because constant disconnect and reconnect of
  the sound card which makes sense as the sound settings flashes briefly
  during every popup. Someone on IRC suggested to kill pulseaudio and
  thus restart it, that does not fix the issue. I also tried the Live
  environment thinking it might be due to some old config files in home
  partition (which was preserved during upgrade), but this problem
  happens in the live environment as well, which makes sense because it
  was present during installation as well.

I have the same issue with Ubuntu 16.04 (LTS) on my workstation. 
The sound bar regularly pops up when I am listening to music, both when using 
speakers or USB headphones.

Hi, I have the same issue with my brand new Ubuntu 16 upgrade. 
With me the sound bar is popping up only when the headphones are plugged in. 
When they are not, everything is fine.

Have the same issue on 16.04 (kernel 4.4.0-66) with a Realtek ALC892
card. acpi_listen shows multiple headphone plug/unplug events:

jack/headphone HEADPHONE plug
jack/headphone HEADPHONE unplug

The volume dialog popping up is a consequence of the system thinking a
jack was plugged in. Moreover, I find that once I plug in headphones,
the spurious jack events stop. Is this a Pulseaudio bug, or something in
ACPI/further upstream?

Hi. I have the same problem with my installation. I'm using Ubuntu 16.04. I get 
the same popping sounds as other users. It seems like that the sound goes very 
high for a split second every minute or so and I get the volume notification 
bubble with it.
It also happens when I mute the sound. In that case it unmutes the sound and 
stays unmutted.
Has anybody with understanding of the inner workings of the sound system 
identified the problem yet?
This wasn't an issue for me in Ubuntu 14.04.

dualshock3nerd's solution also works for me.  It may also be worth
noting that the appearance of the bubble is often accompanied by a
slight, but noticeable gap in audio playback, which sounds like a click.
(I can tune out the bubble, but the click demands attention!)  Seems
like the problem is pretty well understood at this point, though, so I
won't post any logs unless requested.

Bug description:
  I upgraded to Ubuntu 16.04 yesterday, I did a clean install while
  preserving the home directory which is mounted in a separate
  partition. Even during the installation I had constant volume
  notification bubble appearing on the top-right corner of my screen, it
  would come and go about twice every minute. I just thought it was due
  to installing hardware drivers or something but the problem persisted
  even when I booted into the freshly installed system. I did some
  searching and two users reported the exact problem. And one more
  suggested this happens because constant disconnect and reconnect of
  the sound card which makes sense as the sound settings flashes briefly
  during every popup. Someone on IRC suggested to kill pulseaudio and
  thus restart it, that does not fix the issue. I also tried the Live
  environment thinking it might be due to some old config files in home
  partition (which was preserved during upgrade), but this problem
  happens in the live environment as well, which makes sense because it
  was present during installation as well.

I found a more viable workaround based on comment #18 by Anthony that
still lets you use your headphone jack provided you have an unused front
microphone jack.

Install alsa-tools-gui
Run hdajackretask
Override "Green Headphone, Front side" to "Not connected"
Override "Pink Mic, Front side" to "Headphone"
Click "Apply now"
Click "Install boot override" to make this permanent

>From now on, just plug your headphones in the front microphone jack instead of 
>the front headphone jack and you're golden. Speakers still get automatically 
>muted when you plug in your headphones.
This is much more viable to me as I just use a USB microphone anyway.
I tested this for a few days and this bug never reappeared.

I had exactly the same problem (ubuntu 16.04 latest). Just started
happenning, no apparent cause or changes beforehand.

Tried all the usual suggestions: eg:

a) rm -r ~/.config/pulse; pulseaudio -k - reboot
b) Open VLC,Go to Preferences,then to Sound,click in the reset preferences 
button,Close VLC
c) sudo alsa force-reload
d) alsamixer - auto-mute - disable

- worked for a little while,but then came back again.

Just done Anthony's workaround above (#18) - & so far it has worked
(thanks Anthony). just have to remember how to re-enable it agan
whenever i want to use headphones in future.

Seem to recall this same/similar problem plagued users on Ubuntu 12.04
for a while.

We seem to be going backwards a little with 16.04. Looking forward to a
fix.

Found a workaround that worked for me. I've seen a lot of this issue
just googling around and a few different threads lead to "hda-jack-
retask" (evidently a part of alsa-tools-gui now) as a fix. Install alsa-
tools-gui, run "hdajackretask" and switch the "front green jack" to "not
connected"

It's not a perfect fix but my system is an HTPC and that jack doesn't
get used anyway. Hope this helps.

Title:
  Volume notification keeps popping up constantly(the notifyosd bubble
  on top-right corner)

Status in pulseaudio package in Ubuntu:
  Confirmed

Bug description:
  I upgraded to Ubuntu 16.04 yesterday, I did a clean install while
  preserving the home directory which is mounted in a separate
  partition. Even during the installation I had constant volume
  notification bubble appearing on the top-right corner of my screen, it
  would come and go about twice every minute. I just thought it was due
  to installing hardware drivers or something but the problem persisted
  even when I booted into the freshly installed system. I did some
  searching and two users reported the exact problem. And one more
  suggested this happens because constant disconnect and reconnect of
  the sound card which makes sense as the sound settings flashes briefly
  during every popup. Someone on IRC suggested to kill pulseaudio and
  thus restart it, that does not fix the issue. I also tried the Live
  environment thinking it might be due to some old config files in home
  partition (which was preserved during upgrade), but this problem
  happens in the live environment as well, which makes sense because it
  was present during installation as well.

First time posting on launchpad. Let me know if I missed something. :)

I am having this issue as well. Volume slider notification keeps popping
up. pulseaudio log makes it seem like the same jack plugged/unplugged
event.

Did anyone come up with a good workaround until this is resolved? A way
to mute the pulseaudio notifications?

I'll try and run a log-file capture when I have some time. The problem
occurs for me with both headphones in either front ports on the case,
directly to the rear motherboard ports, and with the sound NOT via
headphones to my monitor. It does seem to pop up more frequently with
headphones connected though.

I also have the same problem. Audio from my Blue microphones snowball is
distorted (slowed down) as well. It's *really* annoying.

Title:
  Volume notification keeps popping up constantly(the notifyosd bubble
  on top-right corner)

Status in pulseaudio package in Ubuntu:

Bug description:
  I upgraded to Ubuntu 16.04 yesterday, I did a clean install while
  preserving the home directory which is mounted in a separate
  partition. Even during the installation I had constant volume
  notification bubble appearing on the top-right corner of my screen, it
  would come and go about twice every minute. I just thought it was due
  to installing hardware drivers or something but the problem persisted
  even when I booted into the freshly installed system. I did some
  searching and two users reported the exact problem. And one more
  suggested this happens because constant disconnect and reconnect of
  the sound card which makes sense as the sound settings flashes briefly
  during every popup. Someone on IRC suggested to kill pulseaudio and
  thus restart it, that does not fix the issue. I also tried the Live
  environment thinking it might be due to some old config files in home
  partition (which was preserved during upgrade), but this problem
  happens in the live environment as well, which makes sense because it
  was present during installation as well.

Clean install, 16.04 64 bit...volume indicator keeps popping up. Also,
the volume position does not agree with my system volume position.

System volume may be at 50%, the volume indicator that keeps popping up
is around 80%.

I have similar problem. After every start system, in top right corner
displaying Volume Notification (Mute), but once.

Title:
  Volume notification keeps popping up constantly(the notifyosd bubble
  on top-right corner)

Bug description:
  I upgraded to Ubuntu 16.04 yesterday, I did a clean install while
  preserving the home directory which is mounted in a separate
  partition. Even during the installation I had constant volume
  notification bubble appearing on the top-right corner of my screen, it
  would come and go about twice every minute. I just thought it was due
  to installing hardware drivers or something but the problem persisted
  even when I booted into the freshly installed system. I did some
  searching and two users reported the exact problem. And one more
  suggested this happens because constant disconnect and reconnect of
  the sound card which makes sense as the sound settings flashes briefly
  during every popup. Someone on IRC suggested to kill pulseaudio and
  thus restart it, that does not fix the issue. I also tried the Live
  environment thinking it might be due to some old config files in home
  partition (which was preserved during upgrade), but this problem
  happens in the live environment as well, which makes sense because it
  was present during installation as well.
