@Devid The issue reappeared and I tried everything you suggested. Here are the 
observations/outputs:
- Sound icon doesn't change while receiving an SMS
- I tried step 4 of your original suggestion (adjust volume after typing a 
number in dialer-app). While typing numbers dialer-app produced dialpad-tone as 
usual. But pressing volume buttons after that only changed the "speaker volume" 
which was already high.
- I seldom use apps which use audio.
- Output of 'pactl list sink-inputs' are attached herewith.

On trusty systems (at least), walinuxagent ships the udev rules to
produce the devices that cloud-init expects to find.  So that isn't the
source of this bug.

Instead, the problem is that the Azure data source defaults to using
/dev/sdb for ephemeral0 ('disk_aliases': {'ephemeral0': '/dev/sdb'} at
[0]).  Iff we detect a _fabric-formatted_ (i.e. NTFS) ephemeral disk,
then the data source updates this default to instead point at that
ephemeral disk (which will, correctly, be /dev/disk/azure/...).  This
happens fine on every first boot, but on subsequent boots, we don't find
a fabric-formatted ephemeral disk (because we reformatted it on first
boot), so we don't update the default, so we end up rewriting the mounts
to point at /dev/sdb.

(I'll give fixing this some thought, and then comment again with
suggestions.)

Kernel log from 4.7.0 trying to boot

** Description changed:

  Booting with Nvidia GTX 970 hangs with an orange screen in kernels 4.7
  and 4.8rc1 (but works with 4.4.0).
  
  I've tried:
-   * Plain yakkety 4.4.0-34-generic
-   * Mainline kernel 4.7
-   * Mainline kernel 4.8rc1
+   * Plain yakkety 4.4.0-34-generic
+   * Mainline kernel 4.7
+   * Mainline kernel 4.8rc1
  and only the first one works. Kernels 4.7 and 4.8rc1 freeze with a bright 
orange screen.
  
  Best I can tell, the frozen orange screen coincides with kernel log
  messages:

I tried stock 16.04, and upstream xorg; no improvement.

What I've identified is: any xorg version that supports DRI3 shows a
performance regression even if only DRI2 is active. Enabling DRI3
worsens video performance and tearing.

xorg-lts-vivid is the latest version that will work flawlessly.

I installed muthbuntu on a brand new WD Blue 3TB HD, but it could not
boot, saying something about faile to read/write hd0. I tried boot
repair, but no luck. So I tried re-installing with erasing the hd, but
received this error. A couple of wierd things, the HD was labeled as
sde, even though it was in the first port. and there was always a 1MB
free space partition that could not be used at the begining of the disk.

The command 'sudo lsusb -v' do not solve the problem.

The problem persist...

It works just only one time in the USB 3.0 input, but once disconnected
and re-connected, Ubuntu no longer recognizes it.

Hi, I can't make much out of your screenshot. It just looks like a big
white rectangle to me.

Could you run this command so we can at least see what your nautilus
settings are?

Do you still have the problem if you don't have nautilus draw desktop
icons?

If you have Unity installed, could you check whether you experience it
on Unity too?

Virtually all of GNOME 3.20 is in Ubuntu 16.10 (development) so you're
welcome to try out an alpha image to see if you can reproduce these bugs
there.

Unfortunately, adding translator comments is not quite sufficient.  If
it were only that simple, I would submit a patch.  And it has been two
years now, counting this bug report.

The problem is that unity-control-center somehow gets several strings
(usually titles) from /other/ programs such as deja-dup, language-
selector, and undoubtedly many more.  It is very time consuming for
translators to figure out where these problematic strings come from, and
it is not even a solution to fix it in the other program: If we add a
hyphen in "Sikkerhedskopier" ("Backups"), so it becomes "Sikkerheds-
kopier", then it will be correct in the control center but wrong
/everywhere/ else in Deja-dup, and that string is used in very many
places where the hyphen would introduce an error.

Since unity-control-center does not handle long strings well, it is
unity-control-center's fault.  It must define its /own/ strings and have
proper translator comments so the translators know where to place
hyphens.  And probably it should allow longer strings, too, and do
linebreaks automatically.  Even in the middle of the words if it needs
to, because ugly is better than absent. Now it simply refrains from
rendering the whole string.

This has been broken in many languages for years, and it's a really
basic part of the UI which every user sees, so this is a serious
internationalization problem.  Please, I implore, give it the priority
it deserves.

A calibration step in libsane's genesys_gl124.c measures the average
illumination (line ~3062) and then uses that value in a division on line
~3090.

The divisor would probably never be zero if your scanner is working, but
if the LEDs are not turning on then all bets are off!

I plugged my scanner into a different USB port, and that fixed it. I
suspect it is a USB power supply issue preventing the LEDs from working.

When I download something in Firefox and it completes, the blue arrow
animated image shows and stays over the toolbar indefinitely.  As other
things change (causing parts to rerender) in the affected area, that
portion of the image goes away.

This occurred in FF 47 and now 48 too.

It's not only third party apps that isn't showing.. The most apps I used
and installed from old ubuntu-software-center doesn't exist in the
"gnome-new-one"... I can list more than 50 "simple" (not for advanced
users Matthias mentioned) apps that exists in the old one, installed
normally on ubuntu 16.04 and didn't and don't exist on the new one. I
also did a clean install, but problem remains. It is like the new
software center reads a different repository than synaptic, apt, apt-get
and old software center..

It isn't an "opinion".. I also use old one or synaptic as a solution...
but it is a problem!

Your Thinkpad X230 has Intel HD Graphics 4000, and I think you're
experiencing the same intel graphics bug (or intel graphics + lightdm
bug) being reported elsewhere.

I have the same problem in Ubuntu 16.04.1 with Intel graphics and
lightdm.
