This is basically the same symptom, but from many years ago:

To reproduce:
1) Suspend (I do so via the dropdown menu)
2) Turn off display. (Not sure if relevant)
3) Turn on display. (Not sure if relevant, problem occurs even if this step is 
after #4)
4) Wake from suspend (I do so by pressing a key on the keyboard OR the power 
button)
5) Screen either stays black or flickers for a second and then goes black
6) Press any key on the keyboard
7) Login screen appears (as if it was in screen-saver/lock mode before)

I'm guessing the bug is not very reproducible because you would have
noticed it already. However, I should note that (aside from a few days
after I installed the OS) the problem happens 100% of the times I wake
from sleep

Some unique details about my setup:
1) The default 20.04 installation failed to a bug installing the packaged 
Nvidia drivers, but skipping that and doing it after installation worked in all 
respects that I can see
2) My computer is custom, not OEM.
3) I do have VMWare Workstation installed
4) I used to use 18.04 and 19.10, neither had the issue

When I navigate to the directory of my "Data" internal drive, right
click, choose properties, and click "open in disks," the disk
application opens to the wrong disk (the first one in the list, my nvme
boot ssd). Additionally, I don't see the drive in the side bar (like
18.04).

If I had tried to do this with a flash drive I was planning to reformat,
I might have accidentally formatted my boot drive.

Very similar to other bugs, may be the same.  I added a new one due to the 
0.122ubuntu8.1 vice just ubuntu8 bit.  I have no idea what any of this means, 
just trying to be helpful if it is a new bug...
Information I think might be important:
I was running Ubuntu Server 16.04 on a Raspberry Pi 3B+.  The pi has an SD card 
with BerryBoot on it, and Ubuntu Server was installed on an external USB Hard 
Drive.  I ran "do-release-upgrade" as recommended by the prompt after logging 
in, as it stated Ubuntu 18.04 was available.  The upgrade process appeared to 
be running smoothly until I received an error that the root device could not be 
found.  I was given Ctrl-C or Enter to Continue options, and I pushed enter.  
The initramfs bug I am reporting came after that.  Not sure if they are related 
or not...

Been lurking on this bug and hoped it would be fixed with the new
4.13.0-17 kernel update, which I just installed after wrestling with the
unreliable apt-get mechanisms for over an hour by the way (but that's a
topic for another bug) but no such luck.  This buggy beast is still
hanging on shutdown with the 4.13.0-17 kernel.

Technically I made it so that any version of PyICU will work but from
what I see of that bug it appears that something has changed in the
parameters.

Removing PyICU completely would break things for the folks who are using
parsedatetime for non-US languages - do you want me to fix this bug with
the most recent version of PyICU (or the version you tell me is needed
for Ubuntu?)

Confirmed this bug with Ubuntu 12.04.2 LTS. Audio becomes unavailable
after HDMI television has been turned off.

dmesg entries:

Running the following to take down the display and throw it back up will get 
everything working again:
 xrandr -display :0 --output DFP2 --off
 xrandr -display :0 --output DFP2 --auto

Replace FDP2 with your appropriate output device. There's probably a
more graceful way to reset the sound but that's beyond my Linux-fu.

I can reproduce the bug following the instructions upon.

After .deb(pluings-ugly/gstreamer0.10-ffmpeg) install online finished. A
error dialog box will be popup, the message is below:

"- An error occurred"
"  Location not found"
It has been proved that the message upon is throw out by totem(source: 3.0.1).

But if I install the .deb(pluings-ugly/gstreamer0.10-ffmpeg) manually,
and then launch totem, it will work smoothly with the wmv file. that
means,  seem the plugins without problem

>From my poor knowledge in this field, seems totem can not realized that the 
>plugin has been updated from (online/command line) installation after it

But totem use gstreamer as it's backend, gstreamer reponsed for all plugins 
initialization.
The problem become more complicated.

My question now is what is the expected behavior after the reproduce
steps.  to reboot the application with a warning message or let totem
play the wmv without any problem without any alert.

Almost same problem. 
I have a laptop LCD with 1440x900 res. As soon as i plug an external CRT 
display with 1680x1050 res. the tray icons disappear from my LCD and are still 
visible on the external display.
Running 12.10 on NVIDIA GT218 [NVS 3100M] graphic controller and nouveau driver

When I was trying to add this source and when I hit reload I got the error msgs

E: Malformed line 54 in source list /etc/apt/sources.list (dist parse)

Go to the repository dialog to correct the problem.
E: _cache->open() failed, please report.

When I close the error dialogue box it shuts down synaptic and when I
reopen synaptic the same error box appears. Also when I try to Open
Ubuntu software Center it won't open. Even after I do a shutdown and
reboot I still get the same errors.

I had to reload Ubuntu to clear the problem

same problem but just with office files .doc .xls .odt and so on. It
seems that firefox-3.5 apparmor profile just allow call to oowriter,
oocalc and so on by default while openoffice.org packages associate
theese document mime types with soffice call.

I got this audit log when the problem occur :

Maybe a patch to openoffice.org-xxx package for mime association is a
better solution

The only thing I've done is removed and reconnected the Front Audio
Connector on the motherboard.

I guess that's done the job then, now ask me how big I feel!

I have the same problem, on a machine with that yields the same lspci
info...

If it helps, In my syslog file these lines always occur together and
only with the hang, which follows immediately mostly, but sometimes as
late as [   78.712010]...

Same problem on my laptop dell D620 with nvidia card. I'm currently using xorg 
nv driver. It was working perfectly on Hardy before upgrading to Intrepid...
I also tried to boot with 2.6.24-21 without any success : blank screen and 
freeze on resuming after suspend.
The debugging procedure at https://wiki.ubuntu.com/DebuggingKernelSuspend 
failed for finding a potentially buggy driver.

I did the following to get linux-image-general in version 2.6.22-14.52 to work 
agaein after an update from an older 2.6.22-14:
- Debugged and replaced my ACPI-DSDT-table according to this how-to: 
http://wiki.ubuntuusers.de/acpi-fix (it workes without, too)
- tried to load speedstep-centrino and acpi-cpufreq with no success
- went into my BIOS and chosen the option "Load optimized defaults" multiple 
times and saved the settings
- after a reboot I re-adjusted the settings (boot range and so on) to my  
preferences

Now, it works again. Maybe, it will help someone here.

Results:

I never had this problem agein. Probably this happened, because plug-ins
(Java, GIJ) changed while FF was running. Maybe this happens if the
Flash-plug-in or Gnash is installed, too.

Once my laptop (with the Ubuntu installation) back from the workshop, I
will try to verify the bug.

The prejudice that Linux is still not supporting much hardware, remains
stubbornly. And you want to help keeping it current? No good idea, I
think.

Support for Laptops is often bad. People owning such a notebook are
being excluded from freq scaling then.

I had luck and a BIOS update.

Please put speedstep_centrino into a package which disables ACPI freq
scaling and tell the people about. With this, you could give this
minority the chance to fix the problem and aren't affecting the
majority. Building such a package wouldn't take much time. But you will
win many friends.

I had the same problem with my Gericom Blockbuster Excellent 7000 259IA7
(Intel Pentium M Dothan 1,6GHz) since a dist-upgrade to Gutsy Gibbon.
speedstep-centrino didn't load.

A bios update to the BIOS from 25.01.2005 solved the problem.
After the update, acpi_cpufreq was loaded by kubuntu itself.

I looked into the synaptic cronic and found out that the problem could be 
caused by removed GIJ-packackes. I didn't know that this were some kind of 
JREs. So maybe firefox wanted to access some missing files from the JRE and 
crashed.
Firefox was just showing a site while synapic did the changes.

While an update was running in Synaptic, Firefox froze.

I switched to Synaptic by minimizing firefox's window. When I wanted to
re-maximize firefox's window, it no longer updated its window.

Hi,

   I recently switch from Edgy to Feisty. My 3D application works fine in 
fglrx-8.28.5, but it has some
problems in 8.32.5. When focusing on the window of this application, it makes 
the own desktop almost black. But when
switching another window of other non-3D application. The whole desktop 
including 3D application displays fine.

   BTW, I am using Radeon X1300 video card.
