Hello Joseph,

I cannot test the issue as I do not have the laptop any more but
unfortunately the issue persits on my new laptop (Dell Latitude E5470):
the working version is 4.4.0-59.80 but with 4.4.0-62.83 and next the
problem comes back. I will post a separate bug for it.

Tested on 4.10.0-041000rc1-generic #201612252031. The problem persists.

There is a regression from 4.4.0-45.66 to 4.4.0-47.68.

While booting I get error:

and booting process fails resuming to BusyBox

The workaround is to unplug and replug the usb stick and the booting
process goes further

As far as I can remember on 12.04 I did not manage to make hardware
accelerated video decoding work at all because of lack of some libraries
/ packages. I think I made it work when I upgraded to 15.10 and this is
when this issue arose.

  SMPlayer with vdpau output plays video ok, but after stopping video in

I tested 4.7-rc1 and it gets even worse. I have not seen

usb 2-1.2: device descriptor read/64, error -110

but it stops booting and unpluging and replugging usb stick does not
make the process continue.

I did the BIOS upgrade.

It has not change anything  - I still get

usb 2-1.2: device descriptor read/64, error -110

and after unplugging and replugging USB stick the booting moves forward.

Christopher,

1) Yes, badblocks scan shows no errors
2) I make bootable USB stick from ISO using dd. I use another USB stick for 
that (not Sandisk). After booting I proceed with installation process - I use 
Sandisk USB as target as if it was a regular harddrive. Installation proceeds 
without problem. Than I boot from Sandisk and this is when the problem occurs.
3) It was few month ago but I think it was ubuntu-15.10-desktop-amd64.iso.

Than I upgraded to 16.04 using update-manager.

Public bug reported:

SMPlayer with vdpau output plays video ok, but after stopping video in
few seconds system freezes. I attach smplayer.log with syslog messages
during crash.

The same happens with Kodi. I attach kodi.log with syslog messages
during crash.


Beside that system works ok, playing video wihout hardware acceleration works 
ok, GPU intensive games work stable.

The problem was there from the first install. Previous kernels tested:
4.2.0-16
4.2.0-18
4.2.0-19
 
There were some errors while installing upstream kernel (I attach thefile)  but 
despite of that it managed to boot. The problem persists.

Walter, thank you for investigating. You will find detailed description
below:

My system is not a fresh 15.10 install. Originally it was 12.04 install
on which I installed self compiled xrdp according to instruction on
http://scarygliders.net/ as original Ubuntu xrdp packet did not work. I
use LXDE desktop for remote sessions. Everything worked fine, including
lxsession-logout. Recently I made few upgrades in a row up to 15.10. And
this is when lxsession-logout started to fail.

If more information is needed just let me know.

lxsession-logout works only once.  When I try to use it again dialog box
does not appear. However two processes apear in the background:
