I also have a Sandisk Sansa Fuze and am receiving the same error with
Hardy:

Just as Evan reported, disabling the ehci_hcd module does work around
the issue:

[ 1125.432269] usb 2-1: configuration #1 chosen from 1 choice

Unfortunately this causes the device to operate in USB 1.1 mode with
much slower transfer rates.

My goal here is to actually use the device in a Windows virtual machine
running inside Hardy.  However, Hardy needs to be able to add the USB
device properly before the VM can use it.  The VM does recognize the
device after I disable ehci_hcd in Hardy, but otherwise it does not.

I can verify this.  Ubuntu 8.04, Firefox 3 Beta 5.  I try deleting all
browsing history, closing Firefox and reopening it, logging out and back
in, etc.  Sometimes it will go there but then sometimes it just doesn't
want to go.  I will look into more details on this.

It's crashing nearly every time I visit www.hulu.com, and not on any
other sites I go to.  The problem would seem to be exactly the same as
skidaddy's.  But if you would prefer, I will just watch this bug to see
its outcome first.

Visually, it does seem like Flash has something to do with it.  The
browser closes right as the Flash applet tries to start (showing a wide
black box at the top of the page).  I am using Adobe's Flash from
flashplugin-nonfree 9.0.124.0ubuntu2.

However, if it doesn't crash, then the Flash works fine -- I can watch
TV shows and everything -- no problems.  It either dies immediately or
it works perfectly.

Description: "Read and write emails"

The reasons are:
- Firefox does not use "Mozilla" in its name, so Thunderbird shouldn't either.
- Thunderbird is pitched by Mozilla as an "e-mail client", not as an "e-mail 
client and newsreader".  (See 
http://www.mozillamessaging.com/en-US/thunderbird/ for proof)  Sure, 
Thunderbird can read news, but so can Evolution, and Evolution's name is just 
"Evolution Mail".
- Evolution's description is simple: "Read and write emails".  This is exactly 
what Thunderbird does, so I think its description should match this.  Other 
items on the GNOME menu have simple descriptions such as this, which don't 
repeat the name of the application.
- If you look at the foreign language translations for the existing menu 
entry...they actually already translate back to the exact strings that I am 
asking you to use above!

I've attached a patch that makes these changes.

I may be experiencing a different bug than everyone here ... but my
problem was solved by a comment in bug 275392.

At a terminal, run:
alsamixer -Dhw

In my case, the PCM level was set to 0%.  I used the right arrow key to
highlight PCM, used the up arrow key to raise the level to 100%, and
then pressed Esc.  This solved my problem.

Gene, the released fix appears to be for the upstream NetworkManager 
package in GNOME, not the Ubuntu network-manager package that is linked 
to the same bug report.  Take a look at the bug page:

NetworkManager is just a tool to control your network connections.  It 
assumes that you already have a network/wireless driver installed 
correctly, which is either compiled in the kernel or is a kernel 
module.  So if you are having a trouble with the kernel driver for your 
network card, you need to get that fixed before you can use NetworkManager.

Now NetworkManager may have a problem *using* your specific driver 
correctly...  in my case, it is simply issuing the wrong command 
(AP_SCAN 1) to wpa_supplicant, when in fact it should be AP_SCAN 2 for 
ndiswrapper.  But this is not (I don't think) a problem with the 
ndiswrapper kernel module.

And bug 39707 is a completely separate issue altogether.

I can confirm that versions -15 and -16 of NetworkManager have *broken
my ability to connect at all to hidden networks*, let alone
automatically re-connect to them after restart/suspend, for both me and
my two roommates' laptops.  We all have different models of laptops but
are all using Gutsy, ndiswrapper, and the Broadcom 43xx chipset.
(ndiswrapper is the version supplied with Gutsy, although I have also
tried using version 1.49 compiled from source, with no better success.)
Using the bcm43xx kernel driver is not really an option, as it is still
quite unreliable from what I have read and from my limited usage.  Also
unhiding the access point is not an option, as we are using Georgia
Tech's campus-wide network (and I know that other college campuses have
a similar setup).  I can barely even Google a site where I can still
obtain the .deb for version -14 of NetworkManager, although that does
seem to allow us connect, albeit requiring us to re-enter the SSID and
WEP key every time we connect.

I have attached my syslog, which matches that of anonym above.  I just
did a completely clean install of Gutsy, then blacklisted bcm43xx and
performed the steps to install/configure ndiswrapper.  After restarting,
I can connect to open wireless networks but not to hidden ones.

I have encrypted my hard disk using the option for doing so in the
installer on the alternate install CD.  When I enter my password at
boot-time at the "sda5_crypt" prompt, the backspace key does not delete
previously entered characters (and the delete key does not work either).
As the installer suggested, I am using a long passphrase for encryption,
so this often gets quite annoying when I make a mistake at the end of
entering the password and have to press "Enter" to start over.

I have a Dell Latitude D600 with Radeon 9000 IGP and am also having
problems.  When Gutsy resumes from sleep (successfully), an error
message appears saying that sleep was unsuccessful.  Then some of the
time, at that point, it will immediately hibernate the system without my
asking.  I have to power the system back on, then it resumes to the same
place, and another error appears saying that hibernate failed (which is
quite untrue, but I don't want it to suddenly decide to hibernate to
begin with after I resume).

I had no issues of this sort with Feisty.  I wiped Feisty to install
Gutsy and haven't done anything crazy with it.

[Bug 50214] NetworkManager + ndiswrapper...

There do seem to be multiple issues here, each one affecting different
drivers.

Regarding the issue affecting ndiswrapper:  I think anonym was onto
something when he noticed that AP_SCAN 1 is being used rather than
AP_SCAN 2.  Can someone come up with a patch so we can see if that is
the problem?  (I don't know where to make the changes to do it
myself...)  Did someone make a deliberate change to NetworkManager so
that it issues AP_SCAN 1 when it used to issue AP_SCAN 2?  What are the
differences between/reasons for using AP_SCAN 1 instead of AP_SCAN 2?
Would a change back to AP_SCAN 2 affect other drivers, possibly breaking
them?

I appreciate everyone's continued effort on this.  I'd really like to
see all of these issues sorted out before Hardy is released with LTS.
In my mind, problematic wireless networking support is one of the
biggest show-stoppers for using Linux out-of-the-box with no prior
experience.

[Bug 50214] Patch to fix NetworkManager + ndiswrapper back to pre-15 state

Okay so to answer some of my own questions...

It appears that the change to AP_SCAN 1 for hidden networks was
deliberate, but this very change is exactly what broke NetworkManager +
ndiswrapper.  I've attached a patch that checks for ndiswrapper and uses
AP_SCAN 2 if it finds it.  Unfortunately it still does not fix hidden
network auto-detection, but at least you can connect to hidden networks
using the "Connect to Other Wireless Networks..." option... which you
couldn't do with -15, -16, or -16.7.10.0 using ndiswrapper.

Which leads me to a very irritating point...

Mark Ferguson's post on 10/23 pointed out that there was a problem with
Alexander Sack's patch, which totally broke ndiswrapper with hidden
networks.  Mark described exactly what the problem was and exactly what
was needed to fix it (change AP_SCAN 1 to AP_SCAN 2 for ndiswrapper).

Over two months later, being frustrated that the problem is going
unnoticed, having never modified a Debian package before or even looked
at the source for NetworkManager, ndiswrapper, or wpa_supplicant
before...I was able to come up with a patch for this in a matter of
hours.  I think anyone who had been working with this code could
probably have done this in five minutes.

I hate to be complaining, and I know that this is a community project
and is enhanced by people in their spare time and all that stuff...  but
seriously guys...  what is going on?  network-manager is a rather
significant package that has become central to Ubuntu's functionality.
And the Broadcom chipset is not uncommon at all (look in Dell laptops,
Linksys wireless adapters, etc...).

Gene, you can see if you are using ndiswrapper by typing the command:
ndiswrapper -l

If it's been set up, you should see at least one line containing "driver
installed".

The only change I made in the patch was to always use AP_SCAN 2 for the
ndiswrapper driver.

I created a similar patch and attached it to bug 50214.  My
understanding from reading the ndiswrapper documentation is that
ndiswrapper is not supposed to work at all with AP_SCAN 1.  For that
reason I set it to always use AP_SCAN 2 with the ndiswrapper driver, as
it does with the orinoco_cs driver.

NetworkManager.  Some observations:

- The problem seems to have nothing to do with AP_SCAN 1 vs. AP_SCAN 2.  I only 
see only place in the entire NetworkManager source that issues AP_SCAN at all 
(supplicant_send_network_config()), but this is not called before the user 
resorts to forcing a connection to a specific hidden network.
- In nm_dbus_get_network_data_cb(), all of my stored hidden networks are 
iterated through, so they at least are being stored somewhere between 
NetworkManager restarts.
- In nm_device_802_11_wireless_get_best_ap(), ap_list seems to be empty -- it 
doesn't iterate through the while loop even once, in the presence of the same 
hidden access point connected to previously.

So without knowing much about how NetworkManager works, and without having more 
time to sit down and study it, I suspect that one of the following is probably 
true:
- The allowed hidden networks are being stored incorrectly.  After 
NetworkManager is restarted, the incorrectly stored parameters for the allowed 
hidden network do not match the actual network parameters, so a connection is 
not made.
- Stored networks are not being added to the ap_list used in 
nm_device_802_11_wireless_get_best_ap() if they are not visible.  What happens 
in between nm_dbus_get_network_data_cb() and 
nm_device_802_11_wireless_get_best_ap() that filters them out()?

Gene, I did some Google searching, and it appears that prism54 also
requires AP_SCAN 2 rather than AP_SCAN 1 to work properly, so you are
probably being affected by the same issue.  I expanded the earlier patch
to fix prism54 in addition to ndiswrapper.  (Disregard the earlier
patch; this one includes its changes).

Here's how you can use the patch to generate a custom package (hopefully
I'm not leaving anything out):

- Download the patch to
/tmp/fix_hidden_networks_ndiswrapper_prism54.patch

- Make sure you have the packages for build-essential, devscripts, and quilt 
installed:
sudo apt-get install build-essential devscripts quilt

- Download the source and update it to version 0.6.5-0ubuntu16.7.10.0:
cd /usr/local/src/
sudo apt-get source network-manager
cd network-manager-0.6.5/
sudo quilt push -a

- Apply the patch:
sudo quilt import /tmp/fix_hidden_networks_ndiswrapper_prism54.patch
sudo quilt push

- Change the version number of the package to distinguish it
sudo debchange --newversion=0.6.5-0ubuntu16.7.10.0custom0
(your editor will appear for you to modify the changelog)

- Build the new version
sudo apt-get build-dep network-manager
sudo debuild binary

- Update your packages
cd ../
sudo dpkg -i libnm-glib0_0.6.5-0ubuntu16.7.10.0custom0_i386.deb 
libnm-util0_0.6.5-0ubuntu16.7.10.0custom0_i386.deb 
network-manager_0.6.5-0ubuntu16.7.10.0custom0_i386.deb

Then see if it works...

NetworkManager should be remembering the SSID for hidden networks, but
it is not.  This is a separate issue that is raised in bug 39707 (which
has also been open for a very long time).  I'd suggest nominating that

Can anyone confirm if this issue is affecting the upstream package in
Debian or GNOME?  If so, we should change the status of that to
something other than "Invalid".  I don't see how this could only be
affecting Ubuntu unless they are using a later version of NetworkManager
(unless changes the Ubuntu maintainers made to this package are
responsible for breaking this in the first place...which could be the
case).

Rama, I am connecting to a network with 64-bit WEP encryption.

The patch I wrote will only fix the issue if you are using the
ndiswrapper or prism54 wireless drivers.  These are both breaking
because NetworkManager is issuing the incorrect AP_SCAN value to
wpa_supplicant.  Beyond that, there are other drivers that seem to be
having different problems.

No, Steve is right.  This is not one problem, but several problems that
have the same apparent outcome, which is making this impossible to fix
if it's treated as a single problem.  Suppose your computer stopped
booting into Ubuntu altogether.  Could it be because your hard drive has
physically failed?  your hard drive's boot sector is overwritten?  your
hard drive's partition table is overwritten?  your BIOS configuration
settings are incorrect?  your BIOS has become corrupt after a firmware
update?  Any of these will produce the same end result.  But if you want
to fix it, you have to focus on and address the cause of the problem at
hand for your particular situation, instead of worrying about all the
different things that could be causing it not to work.

Steve -- my issue relates to ndiswrapper, and Gene's relates to prism54.
Both drivers seem to be fixed by changing the AP_SCAN value that is
issued to wpa_supplicant by NetworkManager when those drivers are
present.(Fixed, meaning that they will connect when the hidden AP
information is entered manually, but they won't connect automatically at
login.)  I haven't tested it in Hardy yet, but I haven't seen anything
in the changelog to suggest that this behavior has been modified at all
from 0.6.5-0ubuntu16.7.10.0?

scubanator87 - the issue here is not automatic connection - it's being
able to connect period to hidden networks.  The issue seems to be that
network-manager is not interfacing with several drivers the way it is
supposed to (using the wrong AP_SCAN value, for example).  I assume then
that there is no problem with ipw2200.

Bug 39707 deals with the automatic connection piece that several of us
are experiencing as well.

@Oliver -- I must respectfully disagree with you that the "login ready"
sound should be on by default for accessibility reasons.

A visually impaired person would not be able to go through the Ubuntu
installation process unassisted -- there are no sounds in Ubiquity.
Whoever is setting up the system for a visually impaired person would
easily be able to enable this sound (or the system manufacturer, in the
case of a pre-loaded system).

In any case, the sound could be much more subtle or inconspicuous and
serve the same purpose.  Instead it sounds tacky, and from an HCI
perspective it makes the Ubuntu startup process abrupt in many contexts
that others have described (library, meetings, late night), rather than
one that is natural and user-centric.

I have to challenge this as a design decision.

Firefox and Thunderbird each use a local NSS database, which is part of
the profile in the user's home directory. It is created when the
application is launched by the user for the first time.

For these applications to automatically load the OpenSC PKCS#11 module,
there would need to be a system-wide NSS security module database, in
which the OpenSC PKCS#11 module could be installed. This has been done
in Fedora but not Ubuntu; see bug #1647285.

This change is not yet in the mainline kernel; feedback is needed to
understand if this fixes the issue on older systems such as yours.

If you are not familiar with patching the kernel, I can create an
(unofficial) ppa for you with a kernel package that contains this patch.
Please let me know if  you are using still Ubuntu Trusty with the
original 3.13 kernel or with an LTS kernel (3.16 or 3.19), or if you are
using a newer release of Ubuntu.

I have two IMAP mailboxes and several calendars.  Before there were many
prompts at startup; now there are only two. (There is only one prompt if
I get rid of one of the IMAP mailboxes.)

code base, using a nightly snapshot or Mercurical?

includes this patch.  This could be an involved process if you have not
done it before; you need to download the latest Thunderbird

version control system; apply the patch from this bug report to the
source code; configure the build system for Lightning; run the build;

i am new to linux, i have the netbook version of ubuntu and this applet
came with it. Sorry to say but the window picker applet task bar
disappears the moment i open anything. The task bar normally shows the
bluetooth icon wireless icon, power icon, logoff icon. etc.etc
Everything disappears. i cannot see anything. I never know what is open
. If i hover over the missing task bar the pop up windows will show
occassionally. ususally the one i dont want.  this is so bad that it
spoils the entire experience of the  linux netbook. whilst i type this
in  the  firefox browser i see no task bar at all. this is a critical
error enough for me to be totally disillusioned. It is a simple task
bar! i guess i have a lot to learn.

There are two upstream commits that make the SOF and SST drivers
mutually exclusive on a specific platform.

The one for Broadwell is now merged into the Ubuntu kernel:

@David Woodhouse: p11-kit-trust.so only handles CA certificates; it does
not pull in the PKCS#11 security module configuration, which is stored
in the system-wide NSS database. This is required where smart cards are
used (using security modules such as OpenSC, either directly or via the
p11-kit-proxy security module). This was part of the issue reported with
this bug.

As it stands now, the PKCS#11 module configuration still has to be
manually added to every Firefox/Thunderbird profile after the
application is first launched by the user and the NSS databases are
created at that time. Changes to the system-wide NSS database won't
propagate (for example, if the system administrator replaces the CoolKey
module with OpenSC).
