Look in the root of the OpenOffice User Profile, enable Viewing of Hidden or 
Archive files, and delete any lock file you find there.  Also, in the folder in 
which you Save your files, also enabling Viewing of Hidden or Archive files, 
delete and lock files.

After applying the recommended format, (###) ###-, and entering
1234567890 the displayed value will be (123) 456-7890.

No idea how/why I typed what's in my original answer.

I just downloaded a new copy of Apache OpenOffice 4.1.11 from Sourceforge.

Can someone tell me what to do about this:
Either another instance of OpenOffice is accessing your personal settings
or your personal settings are locked.
Simultaneous access can lead to inconsistencies in your personal settings.
Before continuing you should make sure user 'Desktop-S7U3OHI/Lhull' closes
OpenOffice on host 'DESKTOP-S7U30HI'.
Do you really want to continue?

--- My previous instance of Windows 10 became corrupt due to a power
failure. Some of my stuff was able to be salvaged, but I don't think I have
a way to get my settings back, or do you know how to do that?  We backed up
the drive but probably didn't restore the right file for OO.

The file is damaged beyond repair, possibly due to over hasty power off of your 
computer or closure of laptop  lid, before the file had been finally written to 
disk from the internal write buffers of the disk apparatus.

a) use Previous Versions (W7 and later) to recover previous versions of the 
file.

b) recover your file as it was when you last opened or saved it, or as it
was when it was last saved with AutoRecovery;

c) find previous versions of the file in the folder it is located in, but
 which have since been deleted;

d) un-delete the temporary files AOO wrote while you were editing the
file, and then deleted.  This may recover your file as it was when you last
opened or you last saved it.
