Someone a while back suggested hitting the lock screen button when the phone 
goes to speaker randomly.  Sometimes that helps.  I, too, have that problem, 
though I haven't noticed it as much after the 11.3 update.

Then there is no other way of stopping it from happening that I know of, it 
is just something people have to deal with if they don't want to change this 
setting.

Yes, that is an option, but then it also means I may have a lot of problems 
turning the speakerphone on when I do want it especially if there is a bit 
of noise around and it is difficult to hear Voiceover in the earpiece.

a 5 SE, you can go into setting/General/Accessibility/VoiceOver/Audio and 
set this to where the speakerphone will not come on during a phone call.

Just make sure auto select speaker in call is set to off.

Make sure your screen is clean and that you don't have a case which might 
overlap the phone to the point where it might confuse the proximity sensor 
which is at the top of the phone.
My 6S Plus seems to have gotten a bit worse with this as well, often when I 
call somebody even if I am holding the phone up to my ear and it's ringing in 
the earpiece as soon as the other person picks up it switches to speakerphone. 
If I let it ring on speakerphone then when the other side picks up it usually 
goes to the earpiece. It's almost like it's switching regardless of whether you 
have it up to your ear or not. I have also observed that sometimes when I am on 
a call as soon as the phone locks it switches to speakerphone and even moving 
the phone away from my ear and back makes no difference unless I first unlock 
the phone. If I know I am going to make a longer call I just use the Earpods. 
There really isn't much you can do except for what I said above and of course 
you can write to accessibil...@apple.com and complain about it.
