When this happens to me I turn off VO and turn it back on either by Siri or 
pressing the power button three times on my iPhone 12 to turn it off and on.

I do also run into this but there is no pattern, sometimes Voiceover just is 
gone, but usually locking the phone, waiting a few seconds and then unlocking 
it makes everything normal again. I never have to reboot my phone in such an 
instance.

What is happening is that you are pressing 3 fingers to the screen instead of 
2. To restore the voice, Either,  press 3 fingers to the screen or turn voice 
over off and back on again. I guess we can fix the problem by changing the 3 
finger gesture to something else. Maybe down the line the team could change it 
to something else that stops this from happening or we can be more careful.

I also either turn voiceover off and then back on again using seri or I use
a triple  finger double tap to turn the speech back on. Sometimes I have to
do that gesture a couple times before voiceover is restored.

I do also run into this but there is no pattern, sometimes Voiceover just is 
gone, but usually locking the phone, waiting a few seconds and then unlocking 
it makes everything normal again. I never have to reboot my phone in such an 
instance.

2. To restore the voice, Either,  press 3 fingers to the screen or turn voice 
over off and back on again. I guess we can fix the problem by changing the 3 
finger gesture to something else. Maybe down the line the team could change it 
to something else that stops this from happening or we can be more careful.

Sometimes while taking my phone out of the case to answer a call I 
inadvertently touch something on the screen that turns the speech off.  A three 
figure double tap might turn it back on but not always on the first try.  I 
wish something could be done to stop this from occurring because it is a little 
aggravating at times.

Hello everybody I can assure you that even when I’ve pressed to finger down 
twice or rather do the double tap after ending a call I lose the speech as well 
and also once I’ve hung up the car if I were to answer the car in the first 
place with my Bose frames tempo for example once I’ve ended the call in the 
same way which is basically pushing the power button once which is the car 
guess what I lose automatically, voiceover so I find that if I tell Siri to 
turn off Voice over and then triple click the home button to turn it back on, 
all seems to be working it’s been like this since iOS 14.5 at least on my end 
and I own an iPhone 8+ and I’m running the latest version of iOS right now. I 
don’t run Beta versions of the software just so everybody is clear on that end.

What is happening is that you are pressing 3 fingers to the screen instead of 2. To restore the voice, Either,  press 3 fingers to the screen or turn voice over off and back on again. I guess we can fix the problem by changing the 3 finger gesture to something else. I have the same issue... though I just turn voice-over off and back on again. 
It takes a few tries though.
