I would say it is not so busy, four or five appointments a day. What is 
interesting in couple of months ago this was never an issue, it started to 
happen with more recent app updates. It can take some time up to three to four 
minutes before I am able to use the app, in fact, I can’t even exit it, so my 
phone is frozen for this time. I reported the issue to the Outlook app team, 
they suggested resetting the account, and if that doesn’t work to delete and 
recreate the account but that did not fix it.

if you go into settings, Calendar, you can set a default alert prior to your 
appointment. This will be used for all new appointments after you have set a 
default alert time.
You can edit your current ones and dig into the details to turn on alerts.
Using Siri is excellent and for reading your appointments, the $4.99 for 
Fantastical 2 is beyond worth the price.

Adds an event with starting and ending time, with and alert 1 hour before
the event.

My conclusion is that start and end are separate,  check how you defined the 
time as a start:time-end:time format.  The above looks verbose, but worth a 
try.

Hi,

In the top right corner of the screen there is an add button. Double tap this.
Now you will be in an edit field where you type in your title.
Now double tap on location which will be in the middle of the screen 
thereabouts.
Type in where it will be.
Now look for something that says starts. Double tap that. Now you will have 
some picker items to do. To use those, double tap on one and then slide your 
finger up or down the screen to change. When you are done with that one, find 
the next one at the bottom of the screen and repeat the gesture. Then in the 
middle of the screen will be the end. Double tap on that to bring the other set 
of pickers up to change your ending time.
Now double tap on done.
The rest is pretty simple. Just slide around and look for repeat and alert. 
When you are done, double tap on done.

You could also use reminders.  Hold down the home key until you hear a 
double tone which indicates that Siri is listening and say, Set a reminder 
for whatever date you want to be reminded.  Siri will respond, what do you 
want to be reminded about?  Double tap just above the home key  and say, 
Dr. appointment with so and so at such and such time.  I usually have my 
reminders of appointments go off the day before the actual appointment.
